86-year-old female
Diagnosis: Pulp necrosis + symptomatic apical periodontitis
Clinical Challenge: Extremely calcified canals under a 50+ year-old crown with good margins and no decay. Extraction was not an option due to the patient’s injectable bisphosphonate therapy (risk of osteonecrosis) and overall frailty.
Access through existing crown and identified 4 highly sclerotic canals.
Conservative shaping using Denco Chameleon Dual Shaper file sequence.
Irrigation protocol:
• 5.25% NaOCl 17% EDTA flush with distilled water in between and as final flush using Irriflex tip.
Sonic activation for enhanced disinfection
Obturation: Bio-C BC sealer with hot modified technique.
